For the purposes of this document, herein after referred to as “Release,” the party intending to participate in the activity run by Orange Willow LLC shall hereafter be referred to as “Participant.” Davidson College, and its trustees, officers, employees and agents, acting within the course and scope of their duties, shall hereafter be referred to as the “College.” The activity run by Orange Willow LLC that the Participant will participate in shall hereafter be referred to as the “Activity.”

In consideration of the Davidson Field Hockey Camp acceptance of the registered camper as the Participant in the Activity for the period in the dates indicated, and in return for the opportunity to participate in this Activity: It is agreed that all risks attendant to watching and/or participating in the Activity including, but not limited to bodily injury, are assumed by the Participant and his/her parents and/or legal guardian and that this assumption is acknowledged, approved, and agreed to by said Participant and his/her parents and/or legal guardian as indicated hereto.

Participant hereby releases and forever discharges the College and Orange Willow LLC from any claim whatsoever that arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Activity. Participant will take for herself or himself any appropriate precautions or medications to treat and/or reduce the likelihood of exacerbating any pre-existing health conditions or allergies.

I hereby certify that the Participant is physically able to participate in the Activity and that I know of no physical impairments which would in any manner limit his/her participation in such a program.

I hereby grant permission for physicians, dentists, other licensed health care providers and their designees employed or directed by Orange Willow LLC to administer outpatient medical, surgical, or dental services as appropriate or necessary antigens or other injections, to perform emergency procedures as necessary or to refer to other duly licensed medical personnel when necessary. In consideration for honoring the Participant’s request to participate in the above activity, I, for myself, my executors, successors, heirs, administrators, and assigns, do hereby release and forever discharge Orange Willow LLC and the College, its respective entities, administrators, coaches, faculty, staff, officers, directors, employees, students and agents from any claims that I might have myself or could bring on the Participants behalf with regard to damages, demands, or any actions whatsoever, including those based on negligence or failure to supervise, in any manner arising out of the Participant’s participation in this activity.

I also hereby agree to save, hold harmless, and indemnify Orange Willow LLC and the College, its respective entities, administrators, coaches, faculty, staff, officers, directors, employees, students and agents, including claims of negligence or failure to supervise, which the Participant might bring against them as a result of his or her participation in the above activity. I recognize that this Release means that I am giving up, among other things, rights to sue the College, its respective entities, administrators, coaches, faculty, staff, officers, directors, employees, students and agents or students for injuries, damages or losses that my child may incur.

By registering for the Activity, I authorize employees, staff or agents to record and edit into video/photograph the name, likeness, image and voice of this Participant and to use all or parts of the video/photograph in print and electronic materials for Orange Willow LLC to promote the Davidson Field Hockey Camp and for no other purpose. Participant will receive no money or remuneration of any kind from Orange Willow LLC related to this grant of permission.
